I don't think there's anyone in Tokyo who buys tissue paper: outside stations and in other busy places, there's often someone handing them out together with some advertising material.
The advertising material doesn't need to be commercial either: here Kitty-chan/Hello Kitty, together with Kenketsu-chan (けんけつちゃん) ask as to become blood donors. (Yes, Kenketsu-chan is the blood donation mascot -this is what her name actually means.)
